Department of Health Western Australia
Each hospital in Western Australia manages their own medical records for each patient they see. If you wish to find out if there is medical or personal information held about you from when you were a child, you must contact a Freedom of Information Officer at the relevant hospital or health service provider. Please visit the Access my WA Health medical records webpage for information about accessing personal medical records.
Access to these records is restricted to protect the privacy of individuals. The Department of Health's website has a list of contacts for each region and the Freedom of Information Officers listed can help with enquiries about records from public hospitals that are now closed.
